The Annual Health and Medical Record form is necessary to participate in all troop outings including Summer Camp

(Please Note: Part C is only required for outings

lasting 72 hours or longer.

i.e. Utah or Summer Camp).

This form is necessary for us to treat your child in the event of accident or injury.

Code of Conduct must be signed by Scout and Parent at the beginning of every year.

Please submit this form along with attached receipts  to Troop Treasurer when requesting reimbursement for Troop expenses.
